What Is Vagal Nerve Stimulation?

Vagal nerve stimulation (VNS) is a neuromodulation therapy in which a small device implanted under the skin of the chest delivers regular electrical pulses to the left vagus nerve in the neck. The vagus nerve has extensive connections to brain regions involved in seizure activity, and regular stimulation can reduce seizure frequency in some patients with epilepsy.

VNS is used as an adjunctive therapy — it is typically added to anti-seizure medications rather than replacing them. It is considered for patients with drug-resistant epilepsy who are not candidates for or have not benefited from epilepsy surgery.

Who May Benefit from VNS

  • Patients with epilepsy whose seizures are not adequately controlled with two or more anti-seizure medications
  • Patients who are not candidates for resective epilepsy surgery
  • Patients with focal or generalized epilepsy syndromes

VNS Programming and Management

VNS programming involves adjusting the stimulation parameters — including output current, frequency, pulse width, and duty cycle — to optimize seizure control and tolerability. Programming is performed using a wand placed over the implanted device.

VNS settings are typically adjusted gradually over time. The device also has a magnet feature that allows patients or caregivers to trigger an additional stimulation burst when a seizure is felt coming on, which may help abort or shorten the seizure in some patients.

CNY Neurological Consulting provides VNS programming and management. Surgical implantation is performed at surgical centers; we coordinate with implanting teams as appropriate.

Frequently Asked Questions

Does CNY Neurological perform VNS surgery?

CNY Neurological Consulting provides programming and management of implanted VNS devices. Surgical implantation is performed at surgical centers. We coordinate with implanting teams as appropriate.

Does VNS eliminate seizures completely?

VNS reduces seizure frequency in many patients, but complete seizure freedom is uncommon. The degree of benefit varies among individuals. VNS is typically used as an adjunctive therapy alongside anti-seizure medications.

How does VNS reduce seizures?

The exact mechanism by which VNS reduces seizure frequency is not fully understood. The vagus nerve has widespread connections to brain regions involved in seizure generation and propagation. Regular electrical stimulation of the vagus nerve is thought to modulate these circuits, reducing seizure frequency in some patients.
